Add a customize Contact Us From to my Vienna-Lite-2-Responsive-Blogger-Template.

First, i need to add a Contact Us From to an specific page in my blog. I have been looking for it everywhere but none of the methods founded have worked for this template. I have added simples contact forms widgets from blogger and they dont even show in my blog.

Also i want to hide the breadcrumbs feature which shows the blog labels like this:
blog->sql->SELECT Statement.
I prefer to sort my post in a different way.

Any feedback would be highly appreciated.
